Minimally invasive headache surgery

Minimally invasive headache surgery is a treatment for patients suffering from chronic headaches. The technique has been known for a long time (>20 years) and widely used in the United States. In recent years, it has also been increasingly used in Europe. Patients suffering from chronic headaches arising at specific sites on the head or neck may be suitable for minimally invasive headache surgery.

The treatment process

Patients are eligible the moment they have had a complete neurological screening including additional examinations such as a CT scan or MRI that rule out other causes of headaches. Next, patients must undergo conservative treatments for their headaches. If these conservative treatments are not sufficient enough, patients are eligible for an intake interview at Goed MC. With this, we work closely with the headache centernederland.nl and all patients are discussed in a monthly multidisciplinary consultation.
At the time patients come to Goed MC for an intake, specific "trigger sites" are examined. These are sites on the head or neck from which the headaches originate. Treatment depends on the presence of symptoms at the time of intake.

1. For chronic headaches with symptoms at the time of intake:
The doctor identifies specific trigger sites responsible for your headaches.
You will receive an injection of a local anesthetic (local anesthetic) at these trigger sites. This helps to reduce the pain immediately and confirms that these sites play a role in your symptoms.

2. For chronic headaches with no symptoms at the time of intake:
In this case, botulinum toxin is injected at the predetermined trigger sites. This reduces muscle tension and prevents the onset of new headaches. Botulinum toxin is long-lasting and can significantly reduce the frequency and intensity of headaches.

The moment the patient responds positively to the above injections, we know that the nerves in this area play a role in causing the headache. At that point, there are two treatment options:

1. A regenerative treatment with platelet-rich plasma:
This is a treatment in which we inject plasma from the patient's own body with a high dose of growth factors around the affected nerve. These growth factors have an anti-inflammatory effect and can calm the irritated nerve.

2. Minimally invasive headache surgery:
This is the surgical technique by which, through a small incision, we locate the affected nerve and completely free it from the tissue surrounding it. This way, the nerve can recover and is less likely to become irritated.
